That’s why, at Maple Heights Behavioral Health, we provide comprehensive aftercare planning as part of each person’s treatment experience. Aftercare planning begins from the moment someone is admitted to our treatment center. Our aftercare planning services are collaborative and personalized, allowing patients to receive a range of continuing care options that can meet their holistic needs. Treatment Approach
When you receive treatment at our hospital, we want you to leave with the resources and support services you need to navigate new challenges and manage potential setbacks. When planning for your continuing care, we will draw on the expertise of a multidisciplinary team, which may include a psychiatrist, a therapist, and one or more members of our nursing staff. During discharge planning, we will also make sure to collaborate with you directly and involve your family if appropriate. There are a range of services we may recommend once someone has completed care at our treatment center. Our recommendations may include outpatient therapy, an intensive outpatient program, or a partial hospitalization program. Whatever your specific needs and circumstances may be, our goal is to help you sustain and build on your progress in our inpatient program. Receiving timely support from caring professionals can make the process smoother and less nerve-wracking. At our hospital in Fort Wayne, Indiana, we offer admissions assessments 24 hours a day, seven days a week. Potential patients can begin the admissions process by calling our admissions specialists or visiting our treatment center in person. We also accept referrals from the professional community and from potential patients’ family members. As part of the admissions process, potential patients complete a thorough assessment.
