The process of planning for a successful discharge from one of our treatment programs begins when a person is admitted to our care. The professionals at our treatment center in LaPlace, Louisiana, are committed to working with each patient to help them achieve their therapeutic goals and plan for the next phase of their healing journey. Discharge planning at River Place involves the patient and a multidisciplinary team, which can include a licensed clinician and case manager as well as psychiatrists, registered nurses, and the patient’s family members. We may also involve referral sources in discharge planning to help ensure continuity of care and the highest levels of support for patients. Our IOP can be an excellent source of ongoing care for adults who need continued professional help in a structured setting. This program serves adults who have mental health concerns and co-occurring addictions using a combination of evidence-based therapy, person-centered approaches, psychoeducation, and crisis intervention methods. Adults in our IOP participate in group sessions three times a week and explore topics like identifying triggers, emotion regulation, and stress. Admissions
