Transition House is an outgrowth of House Calls Total Wellness. An alternative approach to enable individuals to live with wrap around services at home with case directed goals. By utilizing specialists trained in techniques the person can engage without feeling threatened. Coming to the home keeps them comfortable and more willing to meet with a team specialist to promote integration and creation of a lifestyle
All of the services needed for our clients are under one overall Transition House umbrella where we take an interdisciplinary approach. Our team collaborates with the client’s clinical doctors and we advocate on behalf of family.
