Hi, I’m Dr. Josh Gonsher, a licensed clinical psychologist with a warm, down-to-earth approach to healing. I specialize in helping people navigate anxiety, worry, and stress—not by fighting their symptoms, but by creating a new internal landscape where those struggles no longer take center stage. My approach is grounded in Contextual Therapy. I don’t just help clients “fix” thoughts or behaviors—I work with them to build a more meaningful life, one where old patterns and problems naturally begin to lose their grip. It’s about shifting the frame, not just adjusting the picture.

Saturday and Nighttime appointments. Welcome. I’m an LCSW-C practicing in Maryland, and I earned my MSW from the University of Maryland School of Social Work in Baltimore. With almost 40 years of experience in mental health settings, I bring a warm, thoughtful, and grounded approach to helping people better understand what they need and where to begin.
Having worked in mental health across several decades, I’ve seen the field become both richer and more complex. There are now many more kinds of therapists, areas of specialization, and approaches to care than most clients know how to sort through on their own.

Clients choose me as a therapist because they want to overcome feelings of anxiety and shame, build confidence and self-compassion, feel more comfortable with themselves, and function more effectively. After working with me, clients have a toolbox of strategies to cope with urges and behaviors they want to change; understand more about themselves, their emotions, and their behaviors; are able to do things they used to avoid; and can reclaim their time and mental energy from body-focused repetitive behaviors, anxiety, and OCD.

Life is full of transitions- career changes, relationship shifts, identity shifts, relocations, the ending of relationships, or moments when life no longer feels aligned. During these times, it’s common to experience stress, anxiety, self-doubt, low self-esteem, or depression. I work with women who are tired of performing and want support navigating these transitions with honesty and depth. Together, we’ll build insight, clarity, and meaningful change.

I recognize that our world may cause us harm, whether intentional or unintentional. My hope in our work together is to separate any harm experienced from serving as a defining characteristic of your personhood. In my practice, my goal is to honor and uplift your unique light with the hope that you may grow more comfortable in sharing it with others in this world. We all carry an inner light, though adversity can sometimes dim its unique glow. I believe that our bodies are vessels of deep wisdom and discernment — learning to attune to that wisdom can transform how we live.

In therapy sessions, I work with clients to help teach the skills needed to manage, squash, or move on from anxious or intrusive thoughts now and long after we end our time together. You will learn how to become “your own therapist” by learning how to spot anxious or intrusive thoughts and learning EXACTLY what to do, proactively and in-the-moment, to stop them from taking over and keeping you from enjoying life. Clients come to me to for difficulties related to OCD, anxiety, phobias, panic, ADHD, and parent/partner management/support for loved ones with OCD, and perinatal/postpartum anxiety/OCD.

Together, we identify triggers for distress, explore how past experiences influence the present, strengthen emotional regulation, uncover relationship dynamics, and examine behavioral patterns. After understanding the purpose and function of the patterns, we develop and implement practical skills to manage or reduce the impact of anxiety, depression, betrayal, grief, trauma, bipolar disorder and challenges related to identity, roles/responsibilities, impulsivity, addictive behaviors, relationships, and transitions. I take a holistic, developmentally informed approach, addressing the whole person—mind, body, and spirit.

I help patients find hope in adversity. I explore a patient's need for external validation and identify old, negative motivators used to inspire achievement-based goals. I teach self-care and self-regulating practices while modeling healthy attunement. Together, we explore conflict-avoidant behaviors and healthy dependency. Being a highly sensitive person is not a weakness but an empathic strength. I explore the guilt, shame, and fear that cause "analysis paralysis." I analyze unhealthy habits and teach clients to build trust and reconnect with a traumatic body that no longer trusts. You are the expert and I am your guide

Does insurance cover therapy, prescriptions and other behavioral health services?
In general, yes. Laws in the U.S. require insurers to cover essential behavioral health care at the same level as other health care services. Most health plans require a medical diagnosis and treatments that are evidence based and medically essential. This means that life coaching and alternative therapies are not covered, nor is therapy without a diagnosis. Call the number on the back or your insurance card or visit your insurance company's website to review your Summary of Benefits. If you have insurance through your employer, your Human Resources department may also be able to answer questions.
How much does therapy cost with insurance?
The cost of therapy depends on your region, how much training and experience your therapist has, and the type of treatment. The rate for talk therapy generally falls between $100 and $250. For a client seeing an in-network provider, the cost of therapy is often similar to the cost of other health care services. Clients pay co-pays and may need to satisfy a plan’s deductible before benefits kick in.
What is the difference between In-Network and Out-of-Network insurance coverage?
An in-network provider has contracted with an insurer to get paid directly for services, and to accept the insurer’s rate of compensation. Clients are responsible for deductibles and copays. If a therapist is out of network, that means that the client pays the therapist directly for services at the therapist’s regular fee. Out-of-Network providers often give clients a receipt so that they can file for partial reimbursement with their insurance company.
Is it difficult to find an In-Network therapist?
It is not unusual for therapists to limit the number of insurance patients in their practice, so it’s important to check in-network availability when you schedule your first appointment. In regions and times where therapy is in high demand it may take longer to get scheduled with an in-network therapist.
What is the difference between HMO and PPO insurance plans?
HMO health plans only cover in-network services, there is no reimbursement for out-of-network providers. PPO plans often cover both in- and out-of-network services, though the client may pay more for out-of-network services.
What is a Superbill?
A superbill is a receipt for therapy services. It provides all the information required by insurance companies to reimburse you for out-of-network services, including the diagnostic codes for your care, the amount paid, and the dates and duration of care.
Does insurance cover Online therapy?
Many therapists who shifted their practice to online therapy during the pandemic have kept that arrangement. And most insurers continue to cover telehealth services, with a few exceptions. Preventive care (such as an annual depression screening) is often required to be in-person to be covered, and some states require prescribers of controlled substances to see those patients in person.
Does insurance cover Group therapy?
Many insurance plans provide coverage for group therapy provided there is a medical diagnosis for group members, but there are some noteworthy exceptions. For example, Medicare does not always cover group therapy. Group therapy tends to cost less than individual therapy.
Does insurance cover Couples therapy?
Most insurance companies do not cover the cost of couples therapy because there is no medical diagnosis or medically necessary treatment. However, a few plans will offer a limited benefit, so it is still worth checking with your insurance company. Sometimes therapy is covered for a couple when one person has a diagnosis and another family member joins them for joint therapy, but it is considered therapy for the medical condition that includes a family member as a listener or helper, not couples counseling. Without insurance, most clients pay between $100-300 per 60-90 min session for couples therapy. Often clients can find classes or workshops that make it more affordable to work on their relationship. And some employers offer benefit plans that help defray the cost of couples therapy. Learn more about Insurance Coverage and Finding Affordable Therapy
