The families I work with are dealing with caring for an aging or disabled relative. Often the issue is that their loved one can no longer function independently due to physical weakness or cognitive problems due to dementia or Alzheimer's Disease. The precipitating factor is often a hospitalization and rehabilitation in a facility. The family is at a loss as to how much care will be needed upon discharge, what will Medicare pay for, and where will the home care workers come from. I also work with individuals with early Alzheimer's Disease, who need different kinds of services, which is also very stressful for the family.
I meet the family and the person in need of care in the facility, and meet with the staff there to determine what the needs are in order for him/her to return home. I advocate for the patient to get good care in the facility, including extending the stay to complete the physical therapy needed. I interview home care workers to ensure they are compatible.
I have had a full time private practice since 1994, with all of my referrals coming from other clients and professionals. You can read more about my services on my website. Sometimes I provide care management on an ongoing basis for families who live at a distance. I also am a Medicaid expert, so that assets can be preserved.
