DOT SAP EVALUATIONS include a Zoom Platform or Face-to-Face Initial Clinical Assessment to Evaluate Employee. Final Follow-Up Session For Treatment Recommendation Compliance. Follow-Up Testing Schedule and Aftercare Recommendations (as necessary) Written Reports sent to the DER designated employer representative apprising them of referrals, monitoring, and recommendations.
