I don't bill insurance for evaluations, I can provide a superbill to you. After a free 15min interview, if you decide to proceed with the FMLA or SDI application, I require a signed contract and $575 (FMLA ) or $845 (Disability Insurance evaluation) to be prepaid. Results are NOT guaranteed.
