To ensure we have a productive first visit, please be sure to fill out your intake form in advance! During our first visit, we will complete a full mental health assessment and evaluation. There will be lots of questions during this visit, especially regarding your medical and mental health history.
All paperwork such as FMLA and related will only be considered after several sessions of treatment and as clinically indicated. Once we have completed your evaluation, we can then discuss a treatment plan specific to your needs that will get you on the road to feeling better as soon as possible!
