My practice here is based on two fundamental clinical ideas: The first is that in medication management, most psychiatrists are too timid regarding escalation of a medication algorithm, and they're not weighing the risks and benefits to the patient. It's a principal agent problem. My second concern is that as psychotherapists, some (not all, but some) of the core ideas of our therapeutic culture are theoretically flawed, and this makes them counterproductive, or at least less productive than they could be.
