Each year, I review the most important addiction science in medical, neuroscience, and public health journals. In 2025, research addressed questions experts grapple with daily: What are realistic goals of treatment? How should we conceptualize recovery in an era of fentanyl? Are addictions neurobiologically reversible? Where do emerging therapies—such as GLP-1 agonists or ketamine—fit into evidence-based care?
The 15 studies below are unranked and selected for scientific rigor, clinical relevance, and potential to shift thinking/practice in addiction medicine.
1. Rethinking Medication Treatment Goals for Opioid Use Disorder
McLellan and Volkow confront one of the most persistent, emotionally-charged addiction questions: What are acceptable treatment outcomes? We have no cures, so what are we trying to achieve with medications for opioid use disorder? The authors propose a three-stage framework—protection, remission, and recovery. Protection emphasizes survival and overdose prevention; remission focuses on stabilization of cravings/use; recovery encompasses long-term health and functioning.
A person can be in recovery but treated with methadone, buprenorphine, or naltrexone. The authors contend that medication tapering should not be a default goal, and long-term or even lifelong treatment with methadone or buprenorphine may be appropriate for many patients.
2. Fentanyl: Not Just an Opioid Problem
Using national poison center data from 2015–2023, Fitzgerald et al. document a dramatic rise in nonfatal fentanyl exposures involving stimulant co-use. Cocaine-fentanyl exposures increased nearly sevenfold, while methamphetamine-fentanyl exposures rose more than sixfold. The findings underscore a fundamental change in illicit drug use and supply: Polysubstance exposure is the norm. Emergency physicians, addiction specialists, and public health systems must adapt to increasingly complex intoxication and withdrawal syndromes rather than single-substance models. This study reinforces fentanyl contamination as a supply-chain/overdose problem affecting all drug classes.
3. Is Ultra-Processed Food Addictive?
In one of the year’s most provocative papers, LaFata and colleagues argue that addiction to ultra-processed foods meets diagnostic criteria comparable to substance use disorders. Reviewing nearly 300 studies across 36 countries, they showed consistent evidence of craving, impaired control, continued use despite harm, and failed attempts at cutting down. The estimated global prevalence—around 14 percent—mirrors alcohol use disorder.
Neuroimaging and behavioral data demonstrate striking overlaps between food and drug reward circuitry, particularly dopaminergic and self-regulation systems.
Recognizing food addiction may reduce stigma, opening the door to effective treatment approaches, especially with emerging pharmacotherapies.
4. Esketamine Five Years Later—Cautious Promise
Five years after FDA approval, this editorial evaluates the role of esketamine in treatment-resistant depression. While acknowledging its rapid antidepressant effects, the authors highlight unresolved questions about esketamine’s durability, long-term safety, and misuse potential. The rapid expansion of at-home prescribing and online clinics raise concerns about unsupervised use, diversion, and emerging ketamine use disorders.
5. Ketamine’s Antidepressant Effects May Involve the Opioid System
Challenging a long-held assumption that ketamine’s antidepressant effects are solely mediated by NMDA receptor antagonism, Levinstein and colleagues present evidence for synergistic interactions with the endogenous opioid system. This model aligns with clinical observations that ketamine can reduce craving for alcohol and opioids—and helps explain its addictive potential.
6. The Biology of Addiction, Revisited
Eric Nestler’s masterful review synthesizes decades of neuroscience into a coherent biological model of addiction—how repeated drug exposure induces long-lasting molecular and synaptic changes within the mesolimbic dopamine system, nucleus accumbens, and stress circuitry. The adaptations explain craving, compulsive use, and relapse vulnerability. Addiction is not simply learned behavior—it’s a disorder of neural plasticity shaped by gene-environment interactions. Nestler reinforces why relapse risk persists long after detoxification, making long-term treatment strategies necessary.
7. Direct Human Brain Evidence of Reward Circuit Modulation by Tirzepatide
Although limited by design, this study offers a unique window into how GLP-1–based therapies affect craving and motivation. It reinforces the concept that metabolic and addictive disorders share neural substrates—and that pharmacologic interventions may have complex, sometimes paradoxical, effects on compulsive behavior.
In a novel single-subject study using direct electrophysiologic recordings from the human nucleus accumbens, Choi and colleagues show how GLP medications influence reward circuitry.
8. Smoking Cessation Improves Recovery from Other Substance Use Disorders
Analyzing data from more than 2,600 adults, Parks et al. study found that discontinuing cigarette smoking is strongly associated with recovery from substance use disorders. The effect size was striking: Quitting smoking increased odds of SUD recovery up to 42-fold.
This paper challenges the longstanding belief that tobacco cessation should be deferred during addiction treatment. Instead, smoking apparently is a modifiable risk factor for relapse and poor outcomes. For clinicians, the message is clear: Tobacco cessation is no threat to recovery—it’s part of recovery.
9. GLP-1 Agonists as a New Class of Addiction Treatments
Srinivasan and colleagues’ comprehensive review outlines the emerging role of GLP-1 receptor agonists in alcohol and substance use disorders. Preclinical and early clinical data suggest that the medications reduce craving and consumption by modulating reward, stress, and metabolic pathways.
The authors highlight next-generation agents specifically designed for central nervous system penetration. If ongoing trials confirm efficacy, GLP-1 agonists could represent the most important pharmacologic advance in addiction treatment since buprenorphine.
10. Cannabis Use Predicts Tobacco Initiation in Youth
Using longitudinal data from the PATH study, Chen et al. show that adolescent cannabis use significantly increases the likelihood of progressing to regular tobacco smoking. Nearly one-third of adolescents who used cannabis later became regular smokers.
These findings challenge assumptions that declining cigarette use continues independent of rising cannabis use. Clinicians should integrate tobacco prevention messaging into discussions about cannabis, particularly for youth.
11. How Drug Memories Are Encoded in the Brain
Salery et al.’s mouse study identifies specific neural ensembles in the nucleus accumbens encoding cocaine-associated memories. Using the immediate-early gene Arc, the authors demonstrated that distinct neuron populations are activated during memory formation and reactivated during recall.
The implications are profound: Cocaine craving and relapse are encoded in identifiable neural circuits. This work opens the door to targeted interventions disrupting pathological drug memories rather than treating addiction as an abstract psychological phenomenon.
12. The Genetics of Delay Discounting
This massive genome-wide association study by Thorpe et al. identifies 11 loci associated with delay discounting, a behavioral measure of impulsivity strongly linked to addiction. Genetic architecture overlaps with substance use disorders, ADHD, depression, and metabolic traits, establishing delay discounting as a transdiagnostic endophenotype.
This work elevates the ability to delay gratification from a descriptive trait to a biologically grounded risk factor with implications across addiction medicine and psychiatry.
13. Cannabis and the Adolescent Brain
Hurd’s translational review synthesizes human and animal research showing that adolescent cannabis exposure—particularly with high-THC products—can disrupt brain development and increase risks for psychiatric disorders and cannabis use disorder. Importantly, some neurobiological changes may be reversible, highlighting a window for prevention/early intervention.
14. Brain Development Continues into the Fourth Decade
Analyzing diffusion imaging across the lifespan, Mousley et al. identify distinct turning points in brain structural organization at approximately ages 9, 32, 66, and 83. The finding that major maturation continues into the early 30s has implications for addiction prevention, vulnerability, and treatment resistance.
15. Evidence for Medical Cannabis Remains Limited
Hsu et al.’s rigorous review concludes that, despite widespread public support, evidence does not support cannabis or cannabinoids for most medical indications. The authors emphasized careful risk–benefit assessment, drug–drug interactions, and contraindications. For clinicians, the paper reinforces the need for clear, evidence-based guidance amid ongoing commercialization and public misconceptions about “medical” cannabis.
Final Thoughts
The 15 studies reflect neurobiologically grounded, clinically pragmatic research, increasingly focused on survival, quality of life, and long-term outcomes. Addiction research in 2025 moved toward evidence offering better tools for understanding, explaining, and treating addictions.
