Anxiety Counselling in Temple

Dr Alastair Ritchie Coomes

It might be a pressure that builds, a relationship that's shifted, or a low hum of anxiety or depression you keep trying to outpace.
I'm a Chartered Counselling Psychologist and UKCP-registered psychotherapist with a doctorate from the Metanoia Institute. I offer longer-term relational therapy for adults and young people across a wide range of presentations. My approach is integrative — shaped around you, at your pace, not a model. I work in person near Blackfriars, EC4, and online.

My clinical experience includes working with depression, social anxiety, relationship issues and low self-esteem.
My goal as a therapist is to help you explore the emotional questions in your life so you can understand, with greater clarity, why you feel like you do. My practice offers a non-judgemental, confidential setting to reflect on what has brought you to this point in your life, and to consider your potential to live differently in the future. Coming from a background in the arts I pay close attention to the narratives we build and the disguises we assume to protect ourselves from pain. Understanding how these patterns are formed can help you decide which aspects of your life you want to hold onto, and which you would rather let go.

Do you feel exhausted and stuck in worries that feel uncontrollable?
Together, we will begin to make sense of what you are going through. Rather than focusing only on reducing symptoms, we will take time to understand what is beneath them. This can help you feel more in control, develop a stronger sense of confidence, clarity and move towards a way of living that anxiety and past experiences may have taken away from you.

Life at times can be challenging, people can often feel isolated and stuck in their struggles, confused about what to do to change the things in life that are troubling them. Reaching out can be hard, trusting others can feel even more difficult. It's at these times that therapy can make a difference. I am a sex-positive, kink-knowledgeable psychotherapist and am familiar working with a range of relationship models and gender dynamics. I work with people from a diverse range of cultural backgrounds, including individuals from straight, queer, poly and kink communities, sex workers, and people who identify across the gender spectrum.

I work well with clients navigating anxiety, identity, burnout, relationship patterns, and questions of meaning.
I’m an experienced psychotherapist working in Central London, with in-person availability in the City and Pimlico, as well as online and from my home in Chiswick. I’ve worked in the NHS and private practice over the past 16 years, offering a warm, relational and affirming space for clients navigating a range of issues. While many people begin therapy to manage symptoms, I support clients in going deeper – exploring attachment patterns, family dynamics, and self-critical thinking. My approach is integrative, drawing on neuroscience, mindfulness, and existential therapy.

I also see people experiencing depression, anxiety, psychosis, addictions, compulsive thoughts and behaviour, eating disorders.
Psychoanalysis is a particular type of talking therapy which values the singularity of each individual. Through an in-depth and detailed elaboration of one's experience, it offers a unique opportunity to confront the specific nature of one's suffering. This process is not always easy, however it can lead to the alleviation of difficult symptoms, changes in interpersonal relationships, or even a radical reorientation of one's life.

When should I seek treatment for anxiety?
Everyone feels anxious at times, but when anxiety causes disruption to your daily life, it may be time to seek help. Such disruption can be mental, physical, or both: having fear dominate your thoughts, avoiding situations or objects that bring on feelings of anxiety, or experiencing shortness of breath, sweating, dizziness, or panic attacks. When these symptoms are recurrent and distressing, it may be time to talk to someone.

What are the most common types of treatment for anxiety?
Talk therapy, particularly cognitive behavioural therapy, is the most common treatment for anxiety. Studies have shown it is equally effective as medication. Other forms of therapy are popular, depending on a patient’s experience. Exposure therapy, for instance, can be useful for people dealing with fears and phobias, or who have experienced traumatic events. Dialectical Behavioural Therapy may be useful for patients who need help with emotion regulation. Anxiolytic medications, like benzodiazepines, can also help with calming a person enough to focus on the skills they’ve learned, and physical exercise has been shown to help symptoms of anxiety across the board.

How long does therapy for anxiety take to work?
The amount of time needed to alleviate symptoms depends on the severity of a person’s condition. Some therapies may take 5-20 weekly sessions to show results. Anti-depressant medications typically take two to four weeks before a benefit is felt. Anti-anxiety medications are felt typically within 20-30 minutes. Medications are best used to offer a patient a better headspace in which to work on techniques learned in therapy. Exercise, improved sleep habits, and meditation are effective, but takes weeks or months to show results.

How effective is treatment for anxiety?
Treatment for anxiety has been shown to be moderately to highly effective, according to a large body of psychology studies and analyses. For some people, treatment using CBT or exposure therapy alone is effective. Others respond best to a combination of medication and therapy.
