After 20 years of working with clients, what I usually hear on the first appointment is fear and self-doubt about one's own ability to get over one's difficulties. Lots of reasons are given to about why they think they can't change including: "I've been ill too long - nobody's been able to help me; how can you possibly help"?, "I have a chronic mental illness and I have been told that nobody can recover", "I have tried to use CBT before and it didn't work - nothing worked", "I'm worse than the other clients you have seen". I will tell you that all these statements are false beliefs. Clients who have said this still changed!
